Preparing for Your First Appointment
Make sure you have filled out both the Health History and Health Assessment Questionnaire forms. (They can be downloaded by clicking on the form names to the right.) The forms can take up to 1 hour to fill out. Please have these completed before your appointment. These detailed forms give important information needed for the complete assessment of your health situation. Initial Appointment
Your first appointment will be up to one hour in length. This is where all your health information is collected. You will be able to describe your health concerns and review the questionnaires you have filled out. You will also have a short physical exam (blood pressure, lung and abdominal exam, etc.) |
